It is Hull City's Anlaby Road ground. They originally played on the adjacent Circle Cricket Ground in 1905, but moved next door a year later. In 1950, they moved to Boothferry Park. They moved back to the old Circle site where the KC Stadium was built and opened in 2002.

Last one.

The club did a MK Dons and moved to this ground in a different town from their original home town. They later moved back to a new stadium in their town of origin.

I was going to say Brighton but probably doesn't fit geographically because they ground shared with Gillingham for a few seasons :!:
